We're constantly on the lookout for organizations that go above and beyond to provide exceptional care and support. We're pleased to introduce you to PneumaCare Health, a leading community health and wellness provider who assists their neighbors with medical, dental, behavioral health, and housing support and guidance across nearly a dozen counties in Northern California, including Colusa, Glenn, and Butte.
Enhanced Care Management (ECM)
PneumaCare is a provider of Medi-Cal's CalAIM Enhanced Care Management services. This program is designed to help Medi-Cal beneficiaries with complex health and social needs, such as those with chronic illnesses, disabilities, mental health conditions, or substance use disorders. ECM goes beyond traditional care coordination by providing comprehensive, person-centered support that addresses the whole person.
Through ECM, individuals are connected with dedicated Community Health Workers who serve as care managers. These care managers work closely with each person to develop a personalized care plan, that can include; advocacy, coordinated healthcare services, medically tailored meals, transportation to medical, dental, and mental health appointments. In addition to recovery resources, day habilitation centers (THRYVE), and more. ECM aims to improve overall health and well-being, reduce unnecessary hospitalizations, and increase engagement in preventive care.

Housing Deposit Tenancy & Sustaining

PneumaCare understands that safe and stable housing is essential for overall well-being. In California, finding affordable housing can be incredibly challenging, and many individuals struggle with housing insecurity and even homelessness. PneumaCare is committed to helping people overcome these challenges and achieve housing stability. They offer a range of housing-related services, including:
Housing Navigation: Navigating the complex housing market can be overwhelming. PneumaCare provides expert guidance and support to help individuals find available housing, understand their rights as tenants, and successfully complete rental applications.
Financial Assistance: Securing housing often requires significant upfront costs. PneumaCare offers financial assistance to help individuals cover expenses such as security deposits, first month's rent, and other move-in costs. This crucial support can make a significant difference in helping people secure stable housing.
Tenancy Support: Maintaining housing can be just as challenging as finding it. PneumaCare provides ongoing support to help individuals address issues with landlords, understand their lease agreements, and build positive relationships with their neighbors. This helps prevent evictions and fosters long-term housing stability.
